One subtraction, and why it matters

Non-HDL cholesterol = total cholesterol − HDL cholesterol. That is the whole calculation, and it requires nothing you don't already have.

What makes it worth doing is what it captures. HDL broadly moves cholesterol away from artery walls; everything else in the total carries it toward them. Subtract HDL and you are left with the sum of every atherogenic particle in one number — LDL, but also VLDL, IDL, remnant lipoproteins and Lp(a). An LDL measurement covers only the largest of those categories.

The convention in longest use is simple: the non-HDL goal sits 30 mg/dL above the LDL goal.

The 2026 ACC/AHA dyslipidemia guideline brought explicit LDL-C and non-HDL-C treatment goals back into use, with LDL-C targets of under 100 mg/dL at borderline or intermediate risk, under 70 at high risk and under 55 for secondary prevention. Non-HDL moved from footnote to co-target.

When non-HDL is the more trustworthy number

Most laboratories don't measure LDL directly — they estimate it with the Friedewald equation, which subtracts an assumed VLDL contribution derived from triglycerides. When triglycerides run high, above roughly 200 and badly above 400, that assumption breaks down and calculated LDL reads falsely low.

Non-HDL has no such weakness: two directly measured values and a minus sign. So a reassuring LDL sitting next to high triglycerides is exactly the situation where this calculation earns its keep.

The same reasoning is why ApoB has gained ground — it counts atherogenic particles directly. Non-HDL is the free approximation of that idea, already hiding in the panel you have.

How do you calculate non-HDL cholesterol?
Subtract HDL cholesterol from total cholesterol. If your total is 210 mg/dL and HDL is 55, your non-HDL is 155 mg/dL. No extra blood test is needed — the two values are already on any standard lipid panel.
What is a good non-HDL cholesterol level?
Under about 130 mg/dL (3.4 mmol/L) is generally considered optimal for adults at average risk. Targets tighten with cardiovascular risk: the long-standing convention sets the non-HDL goal 30 mg/dL above the LDL goal, so a high-risk LDL target under 100 implies a non-HDL target under 130.
Why is non-HDL cholesterol useful if I already know my LDL?
Because it captures every artery-damaging particle rather than just LDL — VLDL, IDL, remnants and Lp(a) are all included. It is also more trustworthy when triglycerides are high, because most labs estimate LDL with the Friedewald equation, which becomes unreliable above roughly 200 mg/dL of triglycerides while non-HDL does not.
Does non-HDL cholesterol require fasting?
No. It is calculated from total and HDL cholesterol, neither of which needs a fasting sample to be interpretable. That is another practical advantage over calculated LDL, which depends on triglycerides and is therefore sensitive to a recent meal.
