Why this ratio is worth two seconds of arithmetic
Insulin resistance pushes triglycerides up and HDL down simultaneously. Because the ratio has one of those on top and the other on the bottom, it moves roughly twice as fast as either number alone — which is why it can look clearly abnormal while both components are still inside their reference ranges.
Research has found it performs well as a surrogate marker of insulin resistance, better than triglycerides or HDL read separately. And unlike HOMA-IR, it needs no insulin test — the two inputs are on every standard lipid panel you have ever had.

Published thresholds land anywhere from about 1.5 to 3.0 in mg/dL depending on the population studied, so this is a signal to investigate rather than a diagnosis. Ratios also run differently across ethnic groups, which is one reason no single cutoff has become universal.
The unit trap
This is the mistake worth guarding against. Triglycerides convert at 88.57 and cholesterol at 38.67, so the ratio does not survive a unit change — a ratio of 3.0 calculated in mg/dL becomes about 1.3 in mmol/L. Anyone comparing an mmol/L ratio against the familiar mg/dL thresholds will conclude they are fine when they may not be. What to do with a raised ratio
- Look at the whole metabolic picture — HbA1c, fasting glucose, waist circumference. A raised ratio rarely travels alone.
